Introduction: Although deep brain stimulation often improves levodopa-responsive gait symptoms, robust therapies for gait dysfunction from Parkinson's disease remain a major unmet need. Walking speed could represent a simple, integrated tool to assess DBS efficacy but is often not examined systematically or quantitatively during DBS programming. Here we investigate the reliability and functional significance of changes in gait by directional DBS in the subthalamic nucleus.Methods: Nineteen patients underwent unilateral subthalamic nucleus DBS surgery with an eight-contact directional lead in the most (...) severely affected hemisphere. They arrived off dopaminergic medications >12 h preoperatively and for device activation 1 month after surgery. We measured a comfortable walking speed using an instrumented walkway with DBS off and at each of 10 stimulation configurations at the midpoint of the therapeutic window. Repeated measures of ANOVA contrasted preoperative vs. maximum and minimum walking speeds across DBS configurations during device activation. Intraclass correlation coefficients examined walking speed reliability across the four trials within each DBS configuration. We also investigated whether changes in walking speed related to modification of step length vs. cadence with a one-sample t-test.Results: Mean comfortable walking speed improved significantly with DBS on vs. both DBS off and minimum speeds with DBS on. Pairwise comparisons showed no significant difference between DBS off and minimum comfortable walking speed with DBS on. Intraclass correlations were ≥0.949 within each condition. Changes in comfortable walk speed were conferred primarily by changes in step length.Conclusion: Acute assessment of walking speed is a reliable, clinically meaningful measure of gait function during DBS activation. Directional and circular unilateral subthalamic DBS in appropriate configurations elicit acute and clinically significant improvements in gait dysfunction related to PD. Next-generation directional DBS technologies have significant potential to enhance gait by individually tailoring stimulation parameters to optimize efficacy. (shrink)

Directional deep brain stimulation contacts provide greater spatial flexibility for therapy than traditional ring-shaped electrodes, but little is known about longitudinal changes of impedance and orientation. We measured monopolar and bipolar impedance of DBS contacts in 31 patients who underwent unilateral subthalamic nucleus deep brain stimulation as part of a randomized study. At different follow-up visits, patients were assigned new stimulation configurations and impedance was measured. Additionally, we measured the orientation of the directional lead during surgery, immediately after surgery, and (...) 1 year later. Here we contrast impedances in directional versus ring contacts with respect to local anatomy, active stimulation contact, and over time. Directional contacts display larger impedances than ring contacts. Impedances generally increase slightly over the first year of therapy, save for a transient decrease immediately post-surgery under general anesthesia during pulse generator placement. Local impedances decrease at active stimulation sites, and contacts in closest proximity to internal capsule display higher impedances than other anatomic sites. DBS leads rotate slightly in the immediate postoperative period but otherwise remain stable over the following year. These data provide useful information for setting clinical stimulation parameters over time. (shrink)

Recent work in cognitive science of religion (CSR) is beginning to converge on a very interesting thesis—that, given the ordinary features of human minds operating in typical human environments, we are naturally disposed to believe in the existence of gods, among other religious ideas (e.g., seeAtran [2002], Barrett [2004; 2012], Bering [2011], Boyer [2001], Guthrie [1993], McCauley [2011], Pyysiäinen [2004; 2009]). In this paper, we explore whether such a discovery ultimately helps or hurts the atheist position—whether, for example, it (...) lends credence to atheism by explaining away religious belief or whether it actually strengthens some already powerful arguments against atheism in the relevant philosophical literature.We argue that the recent discoveries of CSR hurt, not help, the atheist position—that CSR, if anything, should not give atheists epistemic assurance. On occasion footnote material is misplaced--for example, the comment on Strang's article as originally published elsewhere should be on p. 184, not on p. 187, of Volume One. But a greater problem is the fact that almost all the articles on metaphysics and epistemology are by linguistic analysts. Vlastos in his introductory remarks admits as much: "Volume One is heavily weighted on the 'analytical' side." He reports the statement of an un-named advocate of conceptual analysis "that these [analytic] methods now enable us to understand Plato better than he was ever understood by anyone in history--better than by any of his own contemporaries, even better than by himself!" He disclaims such euphoria but insists "there is an element of truth in it after all" insofar as analytic tools can alert a student to ambiguity and its consequences in some of Plato's sentences. Hence, "if we come across such sentences in Plato, it would be plainly true to say that we can understand them better than he did and even to add that we can, therefore, understand him better, since we can see both what he meant to say and the logical liabilities of his incautious sentences." All this is true enough, but a warning still is in order. Linguistic analysts tend to read only some of Plato's dialogues and, even within them, to concentrate on some passages only. This is understandable in view of their interests and the capacity of the analytic technique. But that selectivity might lead them to believe that the Plato thus revealed is the complete and authentic Plato, which it is not. Hence, in reading and teaching Plato one must complement language- and concept-analysis with other approaches. Vlastos himself is aware of this necessity : [[sic]] "Once we have made full allowance for what modern semantics and logic can do to make Plato more intelligible and alive for us today, we should be quick to concede that borrowings from this quarter must be used with economy and discretion, and that continuing reliance on older linguistic and historical disciplines is as essential now as it has ever been in the past if the object of our inquiries is Plato himself, instead of some mock-up more pleasing to current taste." In lieu of an abstract, here is a brief excerpt of the content:Notes and Discussions PARMENIDES THE PROPHET~ The latest word on Parmenides comes from a recent and exhaustive study by Leonardo Tar~n. 1 Among other illuminating and novel interpretations, Tarhn argues that Parmenides was not, after all, guilty of the confusion between the existential and copulative senses of "to be," that he did not identify thinking with Being, and that he had no conception of atemporal reality.~ In these and (...) other respects Tar~n's volume makes a valuable contribution to our understanding of Parmenides. One might judge, however, that Tars work is unusually insensitive to the religious element in Parmenides' poem, and in this respect his analysis seems to echo Reinhardt's unfortunate judgment which calls Parmenides "einen Denker, der keinen Wunsch kennt als Erkenntnis, keine Fessel ffihlt als Logik, den Gott und Geffihl gleichgiiltig lassen." s This inadequacy of TarAn's approach to Parmenides is evident in his treatment of the poem in general, and especially the prologue. The prologue to Parmenides' poem has always been a source of debate. After exploring and rejecting the allegorical interpretation of Sextus (the main source for the proem), Tar~n then systematically examines and rejects the modern interpretations which (among others) make it out to be a shamanistic rapture (Diels, Cornford), Orphic apocalypse (Burnet), religious revelation (Jaeger, Bowra), and mystical experience (Verdenius).4 TarAn judges that Parmenides did not intend his journey to be taken as a reality in any sense, largely on the grounds (1) that Parmenides speaks of his journey as a repeated experience. To take seriously the iterative expressions of the prologue (e.g. fr. 1, lines 1, 3, 8) is incompatible with interpretations which construe it to refer to a specific occasion.6 He supports the conclusion that the proem is nothing more than a literary device with the observations (2) that symbolic interpretations are incongruous with the fact that the goddess expounds her doctrine without symbols, and (3) that the appeal is to Logos, the rational, critical faculty; (4) that the goddess remains anonymous shows that she stands for no religious figure at all ("the 'revelation' is the truth discovered by Parmenides himself"); and (5) that Parmenides could hardly expect us to take seriously the reality of the goddess and other elements in the proem, the reality of which would be entirely incompatible with the thesis of the poem, viz. that reality is One and that all multiplicity and distinction is unreal. ~ z Parmenides: A Text with Translation, Commentary, and Critical Essays (Princeton, 1965). Religion is universal human culture. No phenomenon is more widely shared or more intensely studied, yet there is no agreement on what religion is. Now, in Faces in the Clouds, anthropologist Stewart Guthrie provides a provocative definition of religion in a bold and persuasive new theory. Guthrie says religion can best be understood as systematic anthropomorphism--that is, the attribution of human characteristics to nonhuman things and events. Many writers see anthropomorphism as common or even universal in religion, but (...) few think it is central. To Guthrie, however, it is fundamental. Religion, he writes, consists of seeing the world as humanlike. As Guthrie shows, people find a wide range of humanlike beings plausible: Gods, spirits, abominable snowmen, HAL the computer, Chiquita Banana. We find messages in random events such as earthquakes, weather, and traffic accidents. We say a fire "rages," a storm "wreaks vengeance," and waters "lie still." Guthrie says that our tendency to find human characteristics in the nonhuman world stems from a deep-seated perceptual strategy: in the face of pervasive uncertainty about what we see, we bet on the most meaningful interpretation we can. If we are in the woods and see a dark shape that might be a bear or a boulder, for example, it is good policy to think it is a bear. If we are mistaken, we lose little, and if we are right, we gain much. So, Guthrie writes, in scanning the world we always look for what most concerns us--livings things, and especially, human ones. Even animals watch for human attributes, as when birds avoid scarecrows. In short, we all follow the principle--better safe than sorry. Marshalling a wealth of evidence from anthropology, cognitive science, philosophy, theology, advertising, literature, art, and animal behavior, Guthrie offers a fascinating array of examples to show how this perceptual strategy pervades secular life and how it characterizes religious experience.
